About the role#
This student clinical placement provides an opportunity to gain experience in a hemodialysis healthcare setting. You will work as part of an interdisciplinary team to support patients with kidney disease. All tasks are performed under the direct supervision of a licensed social worker to ensure compliance with company policies and regulatory standards.
What you'll do#
- Assess the psychosocial status, strengths, and specific needs of patients.
- Collaborate with the healthcare team to develop and implement patient care plans.
- Provide psycho-educational counseling to help patients improve treatment adherence and overall quality of life.
- Assist patients in accessing information and forms regarding advance directives.
- Work with financial coordinators to explain financial assistance programs to patients.
- Educate patients and their families on psychosocial issues related to end-stage renal disease.
What you'll need#
- A high school diploma or G.E.D.
- Current enrollment in a social work education program.
- Ability to follow social work policies and maintain accurate documentation.
- A commitment to professional ethics and patient care standards.
- Note: You cannot be a family member of any other employee or patient at the facility.
