About the role#
This clinical placement offers students the opportunity to gain hands-on experience within a hemodialysis healthcare team. You will work under the direct supervision of a licensed social worker to support patient care, contribute to interdisciplinary planning, and learn about the psychosocial aspects of kidney disease. This is a paid, full-time, on-site position with a rolling term.
What you'll do#
- Assess patients to identify psychosocial status, personal strengths, and specific areas of need.
- Collaborate with the healthcare team to develop and implement patient care plans.
- Provide psycho-educational counseling to help patients manage treatment adherence and improve their quality of life.
- Facilitate discussions regarding advance directives and assist patients with the transplant referral process.
- Help patients connect with vocational and educational rehabilitation programs to support their long-term goals.
- Educate patients and their families on psychosocial issues related to end-stage renal disease.
What you'll need#
- A high school diploma or G.E.D.
- Current enrollment in a degree program related to Psychology, Sociology, or Public Health.
- A strong desire to deliver high-quality, supportive patient care.
- Willingness to follow all company compliance programs and policy requirements.

- Location: Kamuela, Hawaii.

- This role requires travel between assigned facilities and various community locations.
