About the role#
This is a clinical placement for students currently pursuing an education in Social Work. You will work within a hemodialysis healthcare team to provide support and care under the direct supervision of a licensed social worker. This role follows an established education plan and focuses on integrating practice based knowledge with professional ethics.
What you'll do#
- Assess the psychosocial status of patients and identify specific needs that affect their treatment outcomes.
- Collaborate with the healthcare team to create and implement patient care plans.
- Deliver psycho-educational counseling to help patients manage treatment adherence and quality of life.
- Assist patients with the referral process for transplant services and community resources.
- Maintain and update resource lists to ensure patients have access to necessary support services.
- Educate patients and their families about psychosocial issues related to kidney disease.
What you'll need#
- A high school diploma or G.E.D.
- Current enrollment in a social work program.
- A strong desire to provide quality care to patients.
- Ability to work effectively under the supervision of a licensed social worker.
- Majors in Psychology, Sociology, Public Health, or Nursing are considered.

- This position is based on-site in Rio Rancho, New Mexico.
- The role is paid and full-time.
- Placements are available on a rolling basis.
