About the role#
This is a paid clinical placement for students pursuing a career in social work. You will function as part of the hemodialysis health care team under the direct supervision of a Licensed Social Worker. The role focuses on integrating your academic knowledge with practice-based experience while supporting patients through their treatment journey. This is an on-site, full-time position with a rolling start date.
What you'll do#
- Assess the psychosocial status, strengths, and needs of patients to support their treatment outcomes.
- Work with the interdisciplinary team to create care plans and identify effective interventions.
- Provide psycho-educational counseling to help patients with treatment adherence and their quality of life.
- Assist with the transplant referral process and provide information on community resources.
- Guide patients through information regarding advance directives and support their discussions with family members.
What you'll need#
- A high school diploma or G.E.D.
- Current enrollment in a social work education program that meets CSWE standards.
- The ability to work under the supervision of a Licensed Social Worker.
- The capacity to provide safe and effective support for dialysis therapy.
- Relevant academic background in Psychology, Sociology, Public Health, or Nursing.
