About the role#
This internship provides practical experience in care coordination for the Spring 2027 semester. You will work directly with Care Coordinators and their supervisors to support patient care in a variety of settings. This is an unpaid position intended for students seeking school credit hours.
What you'll do#
- Shadow Care Coordinators during patient visits, including home visits, phone contacts, and visits to ERs, inpatient mental health facilities, substance use centers, and correctional facilities.
- Help contact clients in outreach to schedule intake appointments.
- Explain program qualifications and services to patients.
- Assist the team with the linkage and referral process.
- Attend community meetings alongside Care Managers.
- Complete administrative tasks as needed.
What you'll need#
- Current enrollment in an Associates or Bachelors degree program in Social Work, Psychology, Mental Health, Substance Abuse Counseling, or a related field.
- The internship must be a requirement of your degree program for school credit.
- You must not have already graduated from a degree program.

- Location: 1000 Young Street, Tonawanda, New York.
- Term: January 2027 to May 2027.
- Modality: On-site.
- Schedule: Placements are available Monday through Friday. Supervisors offer flexible scheduling based on your availability.
