About the role#
This clinical placement offers students an opportunity to integrate academic knowledge with professional practice in a hemodialysis healthcare setting. You will work under the direct supervision of a licensed social worker to support patient care, participate in interdisciplinary team meetings, and contribute to facility quality improvement initiatives. This is a part-time, on-site, paid position with a rolling start date.
What you'll do#
- Assess the psychosocial status of patients and participate in care planning.
- Provide psycho-educational and goal-directed counseling, including support for patients seeking transplant.
- Educate patients and families on kidney disease, treatment modalities, and psychosocial issues.
- Assist patients with advance directives and referrals to community resources.
- Maintain accurate documentation in accordance with company social work policies.
- Help maintain a safe and clean working environment.
- Facilitate staff training on psychosocial topics and assist with patient resource lists.
- Collaborate with the interdisciplinary team on process improvement and quality enhancement activities.
What you'll need#
- A High School diploma or G.E.D.
- A desire to deliver high-quality care to patients.

- Ability to travel between assigned facilities and community locations as required.
