About the role#
As a Social Work intern, you will work under the supervision of the Care Management Supervisor. You will join an interdisciplinary team to improve client access to integrated services. Your work involves collaborating with behavioral health, primary care, and psychiatric staff to ensure clients receive the care they need.
What you'll do#
- Evaluate client status and coordinate treatment activities to address changing needs.
- Direct the client admission process and conduct screenings.
- Provide services for mental health and chemical dependency, including ongoing needs assessment, advocacy, outreach, housing placement, and skill development.
- Coordinate with referral sources to ensure timely follow-up and responses.
- Maintain and review open referrals in clinical records and perform case closures when indicated.
- Collaborate with behavioral health, physical health, and pharmacy staff to support integrated care.
- Attend and participate in multi-disciplinary team meetings.
- Complete agency documentation according to ODMH and best practice standards.
What you'll need#
- Current enrollment in a Bachelor’s or Master’s level Social Work program or an Associate degree internship program.
- Effective verbal and written communication skills.
- Ability to work cooperatively with team members while following organizational values.
