About the role#
This clinical placement provides social work students with hands-on experience in a hemodialysis healthcare setting. You will work as part of an interdisciplinary team under the direct guidance of a licensed social worker. The role focuses on supporting patient care, participating in process improvement, and integrating professional social work ethics into daily practice.
What you'll do#
- Assess patients' psychosocial status, strengths, and specific needs.
- Participate in care planning to help patients meet treatment goals.
- Provide psycho-educational counseling and goal-directed support for patients seeking transplant.
- Educate patients on kidney disease, treatment modalities, and psychosocial issues.
- Assist patients with access to community resources, vocational rehabilitation, and financial assistance programs.
- Provide information regarding Do Not Resuscitate Orders and Advanced Directives.
- Review patient rights, responsibilities, and grievance procedures with patients.
- Maintain updated patient resource lists and provide training to staff on psychosocial topics.
- Adhere to company compliance programs and maintain a safe, clean working environment.

- A High School diploma or G.E.D.
