Med History Pharmacy Intern

About the role#

The Med History Pharmacy Intern supports the pharmacy team by gathering and documenting medication histories for patients admitted to the hospital. Working under the direct supervision of a licensed pharmacist, you will ensure accurate information is recorded in the electronic medical record to support patient care.

What you'll do#

• Obtain medication histories through patient or family interviews, pharmacy outreach, and software queries.
• Enter medication history data into the electronic medical record.
• Communicate the completion of medication histories to the pharmacy team.
• Complete assigned projects as directed, ensuring all tasks comply with legal scope of practice for pharmacy interns.

What you'll need#

• Current enrollment in or graduation from an accredited pharmacy program.
• Active registration as a pharmacy intern with the Minnesota State Board of Pharmacy prior to starting the role.
• Proficiency in medical terminology, drug classifications, and brand/generic terminology.
• Strong communication skills to interact with patients, families, and healthcare staff.
• Ability to work independently, prioritize tasks, and maintain attention to detail.
• Preferred: Students finishing their first or second professional year with the ability to work a minimum of every other weekend and one weeknight per week.

• Schedule: Per diem with rotating weekend shifts.
