Student Social Work Clinical Placement

About the role#

This clinical placement offers students the opportunity to gain hands-on experience within a hemodialysis healthcare team. Under the direct supervision of a licensed social worker, the intern supports the delivery of effective dialysis therapy while adhering to regulatory and company policies. This role focuses on integrating social work theory with practical application to improve patient outcomes and quality of life.

What you'll do#

• Assist in conducting patient psychosocial assessments to identify strengths and needs.
• Participate in interdisciplinary care planning to help patients meet rehabilitation and treatment goals.
• Provide patient education tailored to individual learning styles, including information on kidney disease and treatment modalities.
• Assess patient knowledge of their condition and identify barriers to treatment adherence.
• Collaborate with the healthcare team to facilitate referrals to community resources and support services.
• Assist patients with advance directives and provide information regarding patient rights and responsibilities.
• Support financial coordinators in educating patients about available assistance programs.

What you'll need#

• High School diploma or G.E.D.
• Current enrollment in a relevant field of study, such as Psychology, Public Health, Nursing, Sociology, Human Resources, Health Administration, or Criminal Justice.
• Knowledge of social work theory and methods.
• Strong ability to assess and understand patient needs.
• Outstanding customer service skills and a commitment to delivering superior patient care.
