Student Social Work Clinical Placement

About the role#

This clinical placement offers students the opportunity to work within a hemodialysis healthcare team. Under the direct supervision of a licensed social worker, the intern supports the delivery of safe, effective dialysis therapy while gaining experience in professional social work practices. This role focuses on integrating empirical knowledge with patient care, adhering to company policies, and contributing to quality improvement activities.

What you'll do#

• Assess patients' psychosocial status and identify areas of need to support treatment outcomes.
• Participate in care planning alongside the interdisciplinary healthcare team.
• Provide psycho-educational counseling to improve treatment adherence and quality of life.
• Assist patients in accessing community resources, such as vocational rehabilitation and home health services.
• Educate patients and their families regarding advance directives, end-stage renal disease, and patient rights.
• Collaborate with financial coordinators to help patients understand insurance and financial assistance programs.

What you'll need#

• High School diploma or G.E.D.
• Current enrollment in a relevant field of study, such as Psychology, Nursing, Public Health, or Health Administration.
• Strong customer service skills and a commitment to delivering superior patient care.
• Understanding of psychosocial aspects of patient care.
• Knowledge of company financial policies and procedures.
