Student Social Work Clinical Placement

About the role#

This clinical placement provides students with an opportunity to integrate empirical and practice-based knowledge within a hemodialysis healthcare setting. Working under the direct supervision of a licensed social worker, interns support the mission and core values of Fresenius Medical Care while gaining professional competence in a clinical environment. This is a paid, full-time position with rolling start dates.

What you'll do#

• Assess patients' psychosocial status, strengths, and areas of need to support optimal treatment outcomes.
• Participate in interdisciplinary care planning alongside patients and the healthcare team.
• Provide outcomes-driven psycho-educational counseling to improve treatment adherence and quality of life.
• Assist patients with advance directives and provide information on community resources, such as vocational and educational rehabilitation programs.
• Educate patients and their families on psychosocial issues related to End Stage Renal Disease.
• Collaborate with financial coordinators to help patients navigate financial policies and uncovered services.
• Maintain updated patient resource lists and provide training on psychosocial topics to facility staff as needed.

• High School diploma or G.E.D.

• Ability to work effectively within an interdisciplinary team.
• Note: Interns cannot be supervised by a family member, nor can they provide care for family members who are patients at the facility.

• Modality: On-site.
• This role requires travel between assigned facilities and various community locations.
