About the role#
This student clinical placement provides an opportunity to gain hands-on experience within a hemodialysis healthcare team. Interns work under the direct supervision of a licensed social worker to support patient care, participate in interdisciplinary planning, and assist with psychosocial assessments. This role is designed to foster professional competence, ethical practice, and the integration of social work theory in a clinical setting.
What you'll do#
- Assess patients' psychosocial status, strengths, and needs to support rehabilitation and treatment outcomes.
- Collaborate with the healthcare team to develop and implement patient care plans.
- Provide psycho-educational counseling to improve treatment adherence and quality of life.
- Evaluate patient knowledge of kidney disease and provide tailored education to patients and their families.
- Assist patients with advance directives and provide information on community resources.
- Maintain updated patient resource lists to facilitate referrals for services such as transportation and vocational rehabilitation.
- Support the facility by participating in process improvement activities and quality enhancement initiatives.
What you'll need#
- A High School diploma or G.E.D.
- A strong desire to deliver superior care to patients.
- A commitment to providing outstanding customer service to both internal and external customers.
- Current enrollment in a relevant field of study, such as Psychology, Sociology, Human Resources, or Public Health.
