About the role#
Centerstone is seeking a Masters level intern to join the CSU and WIC teams in Clarksville. This role focuses on supporting clients through the development of comprehensive discharge plans. You will work directly with the discharge planner to ensure clients have the resources they need for their transition.
What you'll do#
- Meet one-on-one with clients in the CSU and WIC programs.
- Develop discharge plans that address mental health, physical health, and substance use needs.
- Connect clients with resources for housing, legal, financial, and social support.
- Coordinate outpatient appointments for psychiatric and counseling services.
- Communicate clearly with team members and clients.
- Follow all company policies and maintain consistent attendance.
- Provide customer service to clients and their families.
What you'll need#
- A Bachelor's degree.
- Current enrollment in a Masters level program for social work or counseling.
- Ability to perform basic electronic medical record tasks.
- Strong problem-solving skills and attention to detail.
- Knowledge of community resources and crisis intervention techniques.
- Ability to de-escalate situations using trained methods.
