About the role#
As a Social Work intern, you will work within an interdisciplinary team to improve client access to integrated services. You will report to the Care Management Supervisor and collaborate with behavioral health, primary care, and psychiatric staff to ensure clients receive the support they need.
What you'll do#
- Evaluate client status and coordinate treatment activities.
- Manage the client admission process and conduct screenings.
- Provide services for mental health and chemical dependency needs, including ongoing assessment, advocacy, symptom monitoring, and housing placement.
- Coordinate with referral sources to ensure timely follow-up and responses.
- Maintain and review open referrals in clinical records and perform case closures when necessary.
- Collaborate with behavioral health, physical health, and pharmacy staff to support integrated care.
- Participate in multi-disciplinary team meetings.
- Complete agency documentation according to ODMH and best practice standards.
What you'll need#
- Current enrollment in a Bachelor’s or Master’s level Social Work program or an Associate degree internship program.
- Ability to communicate effectively in both verbal and written forms.
- Commitment to working cooperatively with team members while following the organization's values.

- This is an on-site role based in Cleveland, Ohio.
- The position is paid and full-time.
