About the role#
The Pharmacy Student Clinical Support Specialist is a remote position for Ohio-based pharmacy students. You will support an interdisciplinary care team by performing deep-dive chart reviews to help inform care for rising risk patients. Working under the mentorship of an Ambulatory Clinical Pharmacist, you will organize and synthesize complex medical histories into concise summaries. This role is designed to complement your pharmacy education during the academic year and requires a commitment of at least 12 hours per week. Please note that time spent in this role does not qualify as internship hours for Ohio pharmacy licensure requirements.
What you'll do#
- Review patient medical records to summarize currently prescribed medications and fill histories.
- Identify and document a patient’s active ambulatory care providers and specialists.
- Summarize recent Admission, Discharge, and Transfer (ADT) events to identify care transition needs.
- Extract and identify patient chronic conditions and key provider recommendations.
- Provide clear, concise written summaries of findings to guide care team priorities.
- Flag medical conditions or social determinants of health for pharmacist review.
What you'll need#
- Completion of at least 2 years (P2) of an ACPE-accredited Doctor of Pharmacy (PharmD) program.
- Familiarity with commonly prescribed medications and the management of chronic conditions like Diabetes, Hypertension, and COPD.
- Proficiency in navigating complex database structures.
- Strong clinical writing skills for a multi-professional audience.
- Prior experience navigating Epic Electronic Health Record systems.
Location & details#
- Location: Remote (must be based in Ohio).
- Commitment: Minimum 12 hours per week, up to a maximum of 19 hours.
- Duration: 6-8 months.
- Compensation: $22.38 to $25.42 per hour.
