About the role#
Bellefaire JCB is hiring a Care Coordinator Intern for the Winter 2027 term. This is a full-time, paid internship based on-site in Medina, Ohio. You will work under the supervision of the CME Clinical Supervisor to learn the fundamentals of intensive care coordination and case management for children and young adults with mental health needs.
What you'll do#
- Observe and learn intensive case management services for youth up to age 21.
- Shadow Care Coordinators as they build and maintain relationships with clients and their families.
- Participate in structured service planning through the High-Fidelity Wraparound model.
- Assist with coordinating referrals and linking families to community resources.
- Learn agency documentation standards, confidentiality requirements, and clinical record keeping.
- Attend team meetings, clinical sessions, and mandatory agency trainings.
- Observe how staff identify crisis situations and escalate them to supervisors.
What you'll need#
- You must be working toward an Associate’s Degree in Human Services or a Bachelor’s Degree in Social Work.
- At least one year of prior experience is required.
- You must have completed the Vroon Vandenburg High Fidelity Wraparound Training.
- A valid driver's license with an approved driving record is required.
