The Medication Management Pharmacy Intern provides support to team pharmacists, patients, and clinicians to ensure effective, safe, and cost-effective drug therapy. This role involves direct patient interaction and clinical support within the health system.
What you'll do#
- Perform data entry, including obtaining medication histories directly from patients and scheduling appointments with pharmacists.
- Document all patient encounters accurately within the health system documentation system.
- Support pharmacists in implementing patient care plans by navigating insurance benefits and assisting with provider communication.
- Provide medication, lifestyle, and disease state counseling.
- Engage patients, families, and authorized caregivers to develop lifestyle behavior changes that positively affect patient health.
- Identify and resolve non-clinical patient medication adherence barriers.
- Coordinate care by notifying pharmacists of prescription needs, confirming prescription pickups, requesting medical records, and sending referrals to other disciplines.
- Enroll patients into criteria-based pharmacy programs.
What you'll need#
- Completion of the first professional year at an accredited college of pharmacy.
- A valid New York State Pharmacy Intern Permit.
- Proficiency in English, both oral and written.
- Previous community pharmacy experience is preferred.
