About the role#
The Medical College of Wisconsin School of Pharmacy is seeking a Population Health PharmD Fellow and Clinical Instructor. This is a full-time, one-year post-graduate position starting July 1, 2027. The role balances clinical practice with academic responsibilities to prepare pharmacists for careers in population health management program design and implementation. You will report to the Population Health Fellowship Director.
What you'll do#
Your time is divided between clinical practice at Froedtert and academic duties. Clinical responsibilities include:
- Practicing as a clinical pharmacist under a collaborative practice agreement.
- Designing and implementing a population health management program for hypertension.
- Improving clinical outcomes through remote patient monitoring.
- Coordinating care across healthcare organizations and optimizing clinical workflows.
- Tracking and communicating operational metrics and quality performance.
Academic and scholarly duties include:
- Designing and delivering educational content within the Doctor of Pharmacy program.
- Facilitating small group-based learning, lecturing, and course coordination.
- Completing the MCW School of Pharmacy Research and Teaching Certificate programs.
- Conducting a scholarly project related to population health.
- Presenting research findings and submitting papers for publication.
- Mentoring pharmacy students and precepting on clinical rotations.
What you'll need#
- A PharmD degree from an accredited U.S. program or equivalent.
- Eligibility for Wisconsin pharmacy licensure, with the ability to obtain it within 90 days of an offer.
- While a PGY1 or PGY2 residency or fellowship is preferred, applicants without PGY1 experience will be considered based on academic performance.
