About the role#
As a Social Work Intern, you will work under the supervision of the Care Management Supervisor as part of an interdisciplinary team. This role focuses on improving client access to integrated services by collaborating with Behavioral Health, Primary Care, and Psychiatric departments to ensure comprehensive care.
What you'll do#
- Evaluate client status and coordinate treatment activities to address changing needs.
- Direct the client admission process, conduct screenings, and support program services.
- Provide mental health and chemical dependency services, including ongoing needs assessment, service coordination, linkage, symptom monitoring, advocacy, outreach, housing placement, and skill development.
- Coordinate with referral sources to ensure timely follow-up and respond to inquiries when clients do not meet service criteria.
- Maintain and review open referrals in clinical records and perform case closures as indicated.
- Collaborate with behavioral health, physical health, and pharmacy staff to facilitate integrated care.
- Attend and participate in multi-disciplinary team meetings and complete agency documentation according to best practice standards.
What you'll need#
- Current enrollment in a Bachelor’s or Master’s level Social Work program or an Associate degree internship program.
- Effective verbal and written communication skills.
- Ability to work cooperatively with other team members.
